Private Individuals

Documents required from a private individual (physical person) for opening the securities account with the custodian Limited Company "KINTO, Ltd".

For non-resident:

• Securities account opening agreement;
• Questionnaire for the securities account;
• Appendix to the agreement (tariffs for services of the custodian);
• Passport or other identification document verifying a person in accordance with the requirements of current legislation;
• Document confirming information on place of residence (home address), 0r temporary residence in Ukraine (in case there is no information on the place of residence in the ID);
• Document with information on citizenship (in case there is no such in ID);
• The document containing details of the bank with the account, with the details of the account (original or duly certified copy);
• The signature of the account administrator specimen card, filled by individual in presence of custodian expert and witnessed by this expert;
• In case the owner of the account appoints the account manager, then to the Questionnaire of securities account the original or notary certified copy of the document, confirming authorities of the account manageris added (agreement of authority, commission agreement, and legalized power of attorney with notary certified translation into Ukrainian language);
• Questionnaire for account manager (individual) (submitted in case there is information in the questionnaire for the securities account on the account manager);
• Questionnaire for account manager (legal person) (submitted in case there is information in the questionnaire for the securities account on the account manager);
• Filled in questionnaire (this is a reading copy of the document. According to the legislation of Ukraine, the document is signed in Ukrainian)
• Application for opening securities account.

If an individual is an entrepreneur, or conducts independent professional activity, the copy of the document confirming that the individual is registered with the state tax authorities (form No. 4-ОПП) should be attached to the questionnaire on securities account.
